Graduate of an approved school of professional nursing

Licensed registered nurse in the state

Minimum of one (1) year of experience as a professional
nurse

It is preferable that the registered nurse has completed a baccalaureate
degree program approved by the National League for Nursing for public health
nursing preparation

Graduate of an approved program of practical nursing

Licensed as a licensed graduate practical nurse in the state where practicing
One (1) year of experience as a nurse

Graduate of a physical therapy school approved by:

The American Physical Therapy Association; or
-
The Council of Medical Education of the A.M.A. in collaboration with
the American Physical Therapy Association; or
-
The Council on Medical Education of Hospitals for the American Medical
Association

Licensed or registered by the state
-
Preference of at least one (1) year of institutional experience

The speech pathologist/audiologist is a member of the American Speech and
Hearing Association and certified by the Association
-
At least a master's degree is required and one (1) full year of supervised
work experience

If required, license of certification by the state

Graduate of a occupational therapy school approved by the Council of Medical
Education of the A.M.A. in collaboration with the American Occupational
Therapy Association or its equivalent

Registered by the American Occupational Therapy
Association

Graduate of a school of social work approved by the Council of Social Work
Education with a master's degree

One (1) year experience in a medical facility (hospital, clinic, rehabilitation
center, etc.) where the team approach to treatment is utilized

A.C.S.W. certification or be in the process of acquiring certification
is preferred, but not required

Licensure by the state, if applicable

Completion of the 9th grade; prefer high school graduate
-
Ability to read and write consistent with job requirements

Completion of a basic aide training program consistent with state and federal
requirements and for which a certificate was obtained

Satisfactory performance on a competency evaluation
-
Has the emotional and mental maturity necessary for establishing and maintaining
a good work relationship with the patient, patient's family, and the personnel
of the Agency

Has received the appropriate PCA training related to the following:

Methodology of assisting patients to achieve maximum self-reliance

Understanding the emotional problem of illness & long term home
care

Procedure for maintaining a clean, healthful and pleasant environment

That all changes in patient condition must be reported to the nurse
-
Maintaining the ethics and confidentiality of patient care
-
Co-operate with the health care team
-
Providing and maintaining appropriate and required records
-
Comprehends the basics of personal care, housekeeping and meal preparation
-
Shows an interest and concern for people
-
Shows good judgment and tact in dealing with the sick
-
Practices good hygiene and is neat in personal appearance
-
Freedom from health problems that may be injurious to patient, self and
co-workers and presents the required documents that indicate such
-
Appropriate work experience
-
Possession of he emotional and mental maturity necessary for establishing
and maintaining a good work relationship with the patient, patient's family,
and the personnel of the
